肠易激综合征发病机制研究进展

摘要:肠易激综合征(IBS)是一种常见的功能性胃肠疾病,其主要表现为腹痛、腹胀及排便习惯改变,但缺乏器质性病变。其发病机制复杂多样,涉及肠道免疫反应异常、肠道菌群失衡、脑 - 肠轴(BGA)功能紊乱等多因素的交互作用。研究表明,肠道屏障受损及局部慢性低度炎症状态是 IBS 发生的重要基础,免疫细胞和炎症因子失衡会导致肠道敏感性及蠕动异常。与此同时,肠道微生态的破坏导致代谢产物异常、短链脂肪酸(SCFAs) 和神经递质变化,从而影响肠神经系统活动及脑 - 肠轴信号传递。应激、焦虑、抑郁等心理因素通过神经内分泌通路进一步加剧肠道功能紊乱,形成神经 - 免疫 - 菌群的互作失衡网络。在治疗策略上,IBS 已逐渐从传统的单一药物治疗向机制导向的多维干预模式转变,包括药物调节免疫与神经功能、饮食及益生菌重建肠道菌群、心理干预改善脑 - 肠轴应激反应等综合管理。综合干预的实施有助于实现从症状控制到机制修复的过渡,提升治疗依 从性和长期疗效。未来研究应加强免疫、菌群及神经系统间的多层机制探讨,发展精准化、个体化治疗策略,为IBS 的长期管理和病理改善提供科学依据。

Abstract

Abstract: Irritable bowel syndrome (IBS) is a prevalent functional gastrointestinal disorder characterized by abdominal pain, bloating, and altered bowel habits, without detectable structural abnormalities. The pathogenesis of IBS is multifactorial, involving abnormal immune activation, disruption of gut microbiota homeostasis, and dysregulation of the brain-gut axis (BGA). Research shows impaired intestinal barrier integrity and chronic low-grade inflammation are considered pivotal factors in disease progression. Dysregulation of immune cells and cytokines contributes to visceral hypersensitivity and motility dysfunction, while microbial imbalance alters metabolic profiles, including short-chain fatty acids (SCFAs) and neurotransmitters, thereby influencing enteric neural activity and brain-gut communication. Psychological stress, anxiety, and depression further exacerbate intestinal dysfunction through neuroendocrine pathways, forming a complex neuro-immune-microbial interaction network. Current therapeutic strategies are shifting from symptom-based pharmacological management to mechanism-driven multidimensional interventions. These include immune and neural modulation through medications, dietary regulation and probiotic supplementation to restore microbial balance, and psychological therapies to stabilize the brain-gut axis. Such integrated, mechanism-oriented approaches aim not only to alleviate symptoms but also to restore gut homeostasis and improve long-term outcomes. Future research should focus on elucidating the cross-talk among immune, microbial, and neural systems, advancing personalized and precision therapies to enhance the long-term management of IBS.
