Abstract

Objective To analyze the current status and existing problems of antimicrobial use during the perioperative period of type Ⅰ incision surgery in a tertiary hospital, and to explore the clinical effects of comprehensive intervention measures on improving the rationality of antimicrobial use. Methods A total of 360 patients who underwent type Ⅰ incision surgeries in this hospital from January 2025 to December 2025 were selected as the research subjects. They were divided into two groups based on the boundary of July 2025, with 180 patients in each group. The subjects in the pre-intervention group adopted the conventional management mode of using antibacterial drugs; the subjects in the post-intervention group were implemented with comprehensive intervention measures. Four observation indicators, including the rationality of antimicrobial use, postoperative infection rate, length of hospital stay, and hospitalization costs, as well as the incidence of adverse reactions to antimicrobial drugs, were compared between the two groups. Results The post-intervention group exhibited significantly higher rates of rational antimicrobial use timing(96.67%), rational duration of drug administration(93.33%), rational drug selection(95.00%), and rational dosage(98.33%) compared to the pre-intervention group(65.00%, 51.67%, 68.33%, and 73.33%, respectively)(P<0.001). The postoperative infection rate in the post-intervention group was 0.56%, significantly lower than that in the pre-intervention group(4.44%)(P<0.05). The average length of hospital stay in the post-intervention group was(6.24±1.35) days, significantly shorter than that in the pre-intervention group(8.76±2.12) days; the average cost of antimicrobial drugs was(326.58±89.74) yuan, significantly lower than that in the pre-intervention group(789.42±215.63) yuan; the average total hospitalization costs were(8 542.36±1 256.78) yuan, significantly lower than those in the pre-intervention group(11 235.79±2 341.56) yuan(P<0.001). The incidence of adverse reactions to antibacterial drugs in the intervention group was 1.11%, which was not significantly different from the 2.22% in the pre-intervention group(P>0.05). Conclusion There are many irrational phenomena in the use of antimicrobial drugs during the perioperative period of type Ⅰ incision surgery in a tertiary hospital. Implementing comprehensive intervention measures can significantly improve the rationality of antimicrobial use, reduce the incidence of postoperative infections, shorten hospital stays, decrease medical cost. It is worth promoting and applying in clinical practice.
