经椎间孔腰椎椎体间融合术治疗腰椎滑脱的疗效分析

Abstract: Objective To explore the efficacy of transforaminal lumbar interbody fusion (TLIF) via the Wiltse approach in the treatment of double-segment degenerative lumbar spondylolisthesis. Methods A total of 40 patients with double-segment degenerative lumbar spondylolisthesis who underwent surgical treatment at Liupanshui Second People’s Hospital of Guizhou Province from April 2020 to June 2024 were analyzed. They were divided into the Wiltse group (21 cases) and the posterior midline group (19 cases) according to the surgical approach, and the clinical effects of the two groups were compared. Results The operation time, intraoperative blood loss, and postoperative drainage volume of the Wiltse group were lower than those of the posterior midline group (P＜0.05). At the 12-month follow-up after surgery, the Visual Analogue Scale (VAS) score for low back pain and Oswestry Disability Index(ODI) score of both groups were significantly lower than those before surgery (P＜0.05), and there were no statistically significant differences in the VAS score for low back pain and ODI score between the two groups (P＞0.05). The postoperative quality of life score of the Wiltse group was higher than that of the posterior midline group (P＜0.05). Conclusion Both TLIF via the Wiltse approach and traditional posterior midline approach can achieve good outcomes in the treatment of double-segment degenerative lumbar spondylolisthesis, but the former has the advantages of being less invasive and enabling faster recovery, which better reflects the concept of enhanced recovery after surgery.
