艾滋病流行病学特征与控制策略进展

摘要:艾滋病(AIDS)至今仍是全球公共卫生领域的重大难题,其流行特征的变迁与防控手段的迭代,始终是学界聚焦的核心。抗逆转录病毒疗法(ART)的广泛应用虽大幅降低了病死率、延缓了病情进展,但人获得性免疫缺陷病毒(HIV)的持续传播态势与防控体系牵涉的多重复杂因素,仍构成不小的挑战。当前全球 HIV 感染呈现明显的多元分化:性接触已成为首要传播路径,男性同性性行为人群感染率长期居高不下;老年群体感染占比逐年上升,因健康认知不足、主动检测意愿低等特点形成新的防控梗阻;地区流行差距悬殊,撒哈拉以南非洲仍承载着全球三分之二的感染负荷。与此同时,病毒基因突变引发的耐药问题(如多替拉韦相关耐药突变检出率上升),叠加不同区域治疗资源可及性的失衡,进一步放大了防控难度。我国《2024-2030 年遏制与防治艾滋病规划》等政策明确提出,需通过强化社区参与、扩大重点人群筛查覆盖面、完善母婴阻断全程管理等综合干预, 实现新发感染数下降、相关死亡病例减少的目标,将疫情稳定控制在低流行水平。但实际推进中,医疗资源城乡与区域分配不均、社会层面对感染者的歧视偏见、部分政策基层落实壁垒等结构性问题,仍制约着防控成效的进一步提升。基于此,本文就艾滋病预防与控制策略的现状及最新进展展开综述。

Abstract

Abstract:Acquired Immune Deficiency Syndrome (AIDS) remains a major global public health challenge, with the dynamic evolution of its epidemiological characteristics and control strategies being a persistent research focus. Although the widespread use of antiretroviral therapy (ART) has significantly reduced mortality and delayed disease progression, the ongoing transmission of human immunodeficiency virus (HIV) and the complexity of prevention systems continue to pose severe challenges. Currently, global HIV infection exhibits multidimensional epidemiological trends: sexual transmission has become the primary route, with notably high infection rates among men who have sex with men, the proportion of infections among middle-aged and elderly populations is rising, creating new prevention challenges, and regional disparities are stark, with sub-Saharan Africa still bearing two-thirds of the global infection burden. Meanwhile, drug resistance due to viral mutations (eg, rising dolutegravir resistance rates) and inequitable treatment access further exacerbate control difficulties. China’s 2024-2030 AIDS Containment and Prevention Plan and similar policy documents emphasize achieving goals through comprehensive interventions like community mobilization, targeted population screening, and mother-to-child transmission blocking. However, structural barriers such as uneven resource allocation, social discrimination, and policy obstacles continue to hinder full progress in prevention and control. This article reviews the current status and advancements in HIV/AIDS prevention and control strategies as follows.
