Abstract

Objective To explore the effect of phased cognitive training based on the Trans-theoretical Model(TTM) on the recovery of cognitive function in patients after cardiac surgery. Methods From August 2025 to May 2026, 100 patients who underwent cardiac surgery in our hospital were randomly divided into the observation group and the control group using the random number table method, with 50 in each group. The control group received routine care, observation group received TTM-based phased cognitive training intervention in addition to the control group. The incidence of delirium, duration of delirium, cognitive function score, sleep quality, anxiety and depression status, quality of life, and nursing satisfaction were assessed and compared. Results The incidence of delirium and the duration of delirium in observation group were both less than control group(P<0.05). The scores of the cognitive function in observation group were higher than control group(P<0.05). After the intervention, the Pittsburgh Sleep Quality Index(PSQI), Hospital Anxiety and Depression Scale(HADS) scores in observation group were lower than control group, and the mos 36-item short-form health survey(SF-36) score was higher than control group(P<0.05). The total nursing satisfaction in observation group was higher than control group(P<0.05). Conclusion Phased cognitive training based on the Trans-theoretical Model can effectively reduce the incidence of post-cardiac surgery delirium, shorten the duration of delirium, promote the recovery of cognitive function, improve sleep quality and anxiety and depression status, enhance quality of life and nursing satisfaction, providing a precise and individualized nursing intervention strategy for the cognitive rehabilitation of patients with post-cardiac surgery delirium.
