骨盆三联截骨术联合股骨近端内翻截骨术在幼年型股骨头骨软骨病治疗中的应用

Objective To analyze the application of pelvic triple osteotomy combined with proximal femoral varus osteotomy in the treatment of Legg-Calvé-Perthes disease(LCPD). Methods A retrospective study was conducted on the clinical data of 33 children with LCPD in our hospital, with the time period ranging from June 2020 to July 2024. The single group(n=12) of children underwent proximal femoral varus osteotomy, while the combined group(n=21) of children underwent pelvic triple osteotomy combined with proximal femoral varus osteotomy. The preoperative and final follow-up X-ray measurement results, Harris score(Harris hip joint function score), and Stulberg hip joint function imaging grading were compared between the two groups. Results At the final follow-up, compared with the single group, the children in the combined group showed a significant increase in the central edge(CE) angle after surgery, a reduction in the Sharp angle, and a significant decrease in the external displacement index(MP)(P<0.05). The Harris scores of both groups increased, and the increase was more significant in the combined group(P<0.05). The excellent rate of the combined group was higher than that of the single group(P<0.05). Conclusion Pelvic triple osteotomy combined with proximal femoral varus osteotomy can increase the CE angle, reduce the MP, and decrease the Sharp angle, improve the Harris score, facilitate the recovery of hip joint function, and is of great significance for the treatment of LCPD.
