腹膜透析患者症状管理体验的现象学研究

摘要

目的 探讨腹膜透析患者在症状管理过程中的主观体验、应对策略及未满足需求,为优化症状管理方案提供依据。方法 采用现象学研究方法,以某三甲医院肾内科接受腹膜透析治疗的12例患者为研究对象,通过半结构化深度访谈收集资料,通过Colaizzi 7步分析法对访谈数据进行主题分析。结果 共提炼出4个核心主题:(1)症状的多维度影响:患者普遍面临腹胀、疲劳、皮肤瘙痒等多重症状叠加,症状对日常生活、睡眠质量及情绪状态均产生显著负性影响;(2)自我管理实践中的困境:包括症状识别不足、家庭支持缺乏以及经济压力导致的治疗依从性波动;(3)医疗支持的个性化需求:患者期望获得个性化症状监测指导、通俗易懂的健康宣教内容及便捷的线上咨询渠道;(4)心理适应的动态过程:多数患者经历从抗拒到接纳的转变,同伴支持在其中发挥了重要促进作用。结论 腹膜透析患者普遍面临多重症状困扰,应关注其主观体验,构建涵盖生理、心理与社会维度的个性化症状管理模式,以改善症状管理效果,进而提升预后效果与生活质量。

Abstract

Objective To explore the subjective experiences, coping strategies, and unmet needs of patients undergoing peritoneal dialysis in the process of symptom management, with the aim of providing evidence to optimize symptom management strategies. Methods A phenomenological approach was adopted. Twelve patients receiving peritoneal dialysis in the Department of Nephrology at a tertiary hospital were recruited. Data were collected through semi-structured, indepth interviews and analyzed using Colaizzi's seven-step method for thematic analysis. Results Four core themes were identified:(1) Multidimensional impact of symptoms: patients commonly experienced overlapping symptoms such as abdominal distension, fatigue, and pruritus, which exerted significant negative effects on daily functioning, sleep quality, and emotional well-being.(2) Challenges in self-management practices: including insufficient symptom recognition, lack of family support, and treatment adherence fluctuations due to financial stress.(3) Individualized needs for medical support: patients expressed strong expectations for tailored symptom-monitoring guidance, accessible and comprehensible health education, and timely online consultation channels.(4) Dynamic process of psychological adaptation: most patients transitioned from resistance to acceptance, with peer support playing a critical facilitating role. Conclusion Peritoneal dialysis patients commonly experience multiple symptom burdens, highlighting the need to consider their subjective experiences. It is therefore essential to develop personalized symptom management models that integrate physiological, psychological, and social dimensions, with the aim of improving symptom control and ultimately enhancing prognosis effect and quality of life.
