医防融合在老年慢性病患者管理中的应用价值研究

Abstract: Objective To explore the value of integrating medical and preventive measures in the management of elderly patients with chronic diseases. Methods From January 2023 to December 2024, 60 elderly patients with chronic diseases were selected as the research objects. They were divided into two groups by random number table method, 30 cases in each group. The control group was given routine management. The observation group applied the routine management + medical prevention integration management mode. Through the comparison between groups, the differences in health cognition, quality of life improvement, medication compliance, self-management ability and psychological state improvement between the two groups were analyzed. Results The scores of health cognition in the observation group were higher than those in the control group after intervention (P＜0.05). The medication compliance of the observation group was higher than that of the control group (P＜0.05). The scores of quality of life ( WHO-QOL ) in the observation group were higher than those in the control group after intervention (P＜0.05). The self-management ability score of the observation group was higher than that of the control group (P＜0.05). The scores of SAS and SDS in the observation group were lower than those in the control group after intervention (P＜0.05). Conclusion The clinical effect of medical prevention integration in the management of elderly chronic disease patients is significant, which can improve patients’ health awareness and intervention compliance, improve their quality of life, facilitate their recovery, enhance their self-management ability, and improve their psychological state. It is worth promoting and applying.
