职业性噪声作业工人高频听力损失分析与预防研究

摘要

目的 分析职业性噪声对作业工人高频听力损失的影响,评估预防性干预措施的实施效果。方法 选取100名接触职业性噪声的作业工人为研究对象,根据是否实施预防性干预措施进行分组,对照组50名实施噪声作业职业健康检查,观察组50名在对照组基础上实施预防性干预措施,比较两组作业工人在干预前后纯音听阈测试结果、高频听力损失发生率、防护依从性、血压变化和心理健康状态。结果 两组工人干预前的3 000、4 000、6 000 Hz频率听阈对比无显著性差异,干预后观察组3 000、4 000、6 000 Hz频率听阈显著小于对照组(P<0.05);观察组的高频听力损失发生率显著低于对照组(P<0.05);观察组的防护依从率显著大于对照组(P<0.05);两组干预后观察组血压值小于对照组(P<0.05);评估工人的心理健康状态,两组干预后工人的焦虑评分和干预前相比均明显降低,观察组干预后的焦虑评分显著小于对照组(P<0.05)。结论 职业性噪声暴露是导致工人高频听力损失的主要风险因素,可伴随耳鸣、血压波动和焦虑等情绪,通过实施系统、综合性的预防干预措施可有效减缓听力下降,降低高频听力损失的发生率,提高作业工人的防护依从性,改善作业工人的血压状况和心理健康水平,对保护噪声作业工人的听力健康有非常重要的临床意义。

Abstract

Objective To analyze the impact of occupational noise on high-frequency hearing loss in workers and to evaluate the implementation effect of preventive interventions. Methods A total of 100 workers exposed to occupational noise were selected as the research subjects and divided into groups based on whether preventive intervention measures were implemented. The control group consisted of 50 workers who underwent occupational health examinations for noise exposure, while the observation group included 50 workers who received preventive intervention measures in addition to the control group. The pure tone audiometry results, incidence of high-frequency hearing loss, compliance with protection measures, blood pressure changes, and mental health status of the workers in both groups were compared before and after the intervention. Results There was no significant difference in the hearing thresholds at 3,000, 4,000, and 6,000 Hz before the intervention between the two groups of workers. After the intervention, the hearing thresholds at 3,000, 4,000, and 6,000 Hz in the observation group were significantly lower than those in the control group(P<0.05). The incidence of high-frequency hearing loss in the observation group was significantly lower than that in the control group(P<0.05). The compliance rate of protection in the observation group was significantly higher than that in the control group(P<0.05). The blood pressure values in the observation group were lower than that in the control group(P<0.05). In the assessment of the mental health status of the workers, the anxiety scores of the workers in both groups decreased significantly after the intervention compared with before the intervention, and the anxiety score of the observation group after the intervention was significantly lower than that of the control group(P<0.05). Conclusion Occupational noise exposure is a major risk factor for high-frequency hearing loss in workers, which can be accompanied by tinnitus, blood pressure fluctuations, and anxiety. Implementing systematic and comprehensive preventive intervention measures can effectively slow down hearing loss, reduce the incidence of high-frequency hearing loss, improve the compliance of workers with protection measures, and improve the blood pressure status and mental health of workers. This has very important clinical significance for protecting the hearing health of workers exposed to noise.
